Provider Request for Doula Services


Thank you for your interest in Community Doula Services. Due to the increase demand for doula services, referrals could see longer wait times as we are providing services to existing clients.

The doula is a professional trained in prenatal care coordination, childbirth attendance, postpartum care coordination, and parenting education through infant age 1. Doulas provide emotional, physical, and educational support to your patient/client throughout her pregnancy and early parenting experience.

The program is provided to your patient/client at no cost and paid for by the Indiana State Department of Health and facilitated by Indiana Minority Health Coalition.

Any Medicaid eligible pregnant or parenting woman with a child under the age of one is eligible.

Please ensure you discussed this program with your patient/client prior to referral. If you have not, please indicate on the referral form. Your patient/client will be directly contacted by a Doula for more information and program benefits.
